TSJH Care Management is a multidisciplinary team responsible for ensuring our patients are admitted and transitioned to the appropriate level of care, have an effective plan of care and are receiving prescribed treatment. The Care Management team is the patient advocate for services and plans needed during and after their stay. Responsibilities: - Clinical Care Coordination - coordinating multiple aspects of care to ensure the patient progresses; - Transition Management - transitioning of the patient to the appropriate level of care needed; - Utilization/Financial Management - managing resource utilization and reimbursement for services; - Outcomes Management - monitoring, and if needed, intervening to achieve desired goals and outcomes for both the patient and the hospital;- Research & Practice Development - Identifying practice improvements and using evidence based data to influence needed practice changes.Requirements: - Graduate of an accredited school of professional nursing. Bachelor's degree BSN is preferred; will consider an Associate Degree in Nursing with case management experience in an acute care setting; - Must be licensed as Registered Nurse with the State of Texa; - 3-5 years experience as a Registered Nurse in an acute care setting; - Case Management experience is preferred; - Proficient in Windows, Word and Excel and will achieve ACM certification within 12 months if not certified. 

The Administrative Director has overall responsibility for the patient care services delivered to patients in IPCU, ER, Respiratory Services and Physical Therapy and assists the CNO in leadership of other patient care departments. The Administrative Director works under the supervision of the Chief Nursing Officer and is responsible for policy review, staff education/orientation and evaluation of patient care standards in all clinical departments. The Administrative Director ensures compliance in the IP department with accrediting bodies and applicable state rules and regulations and CMS. He/she is actively involved in leadership of clinical departments and assures that quality, patient safety and patient care standards are achieved or maintained.  This position also has responsibility for leading department compliance with safety and disaster preparedness. MINIMUM REQUIRED EXPERIENCE: A minimum of five (5) years director-level experience in an acute care setting. Minimum of five years pertinent (med-surg/ICU/OR) clinical experience is required with progressive leadership responsibility. DEGREE/CERTIFICATIONS REQUIRED:  BSN/Registered Nurse in Texas in good standing, BLS certification and ACLS required or obtained within 90 days of hire.  Master’s degree in nursing, business administration or healthcare administration required OR enrolled in graduate school for such degree.

Responsible for ensuring the accuracy and completeness of inpatient and outpatient clinical coding; validates accuracy of coded information and MS-DRG assignment to ensure compliance and proper facility reimbursement.  Also works closely with the Hospital Billing Department and assists with the hospital unbilled report.  Codes final diagnoses and procedures for inpatient admissions and outpatient admissions at TSJH to include: ambulatory surgery, emergency room visits, pain management, medical observation, inpatient surgery, outpatient testing and all other applicable inpatient/outpatient admissions using ICD-10-CM and CPT coding procedures.  Reviews each record to ensure documentation substantiates the code assignments.  Queries physicians when code assignment is not supported by documentation within coding guidelines and compliance.  Abstracts all records with proper physician assignment, dates, admission status, discharge disposition codes and POA status.  Assists with coding final diagnoses and procedures at OPSS for all outpatient admissions using the ICD-10-CM coding classification system and CPT coding procedures.  Successful obtainment and maintenance of the AHIMA credential CCS (Certified Coding Specialist) preferred or successful obtainment and maintenance of AAPC credential CID and or COC.  Will consider experience in lieu of credentials.

This position functions as a system’s analyst in supporting all users and vendors in computer, phone and EMR usage. The right person will understand and lead the hospital in CMS Incentives for Meaningful Use.  This position is responsible for insuring a secure (HIPAA compliant) environment for the hospital’s data.  The department consists of two network administrators and the IT manager.  The manager will assist the network admins in maintaining computer, printer and network equipment.  Position requires 5 years management experience, minimum 2 years hospital IT experience, and bachelor’s degree.  Preferred experience in CPSI (EMR), FUJI PACS, VOIP phone system and Joint Commission standards. 

This position is responsible for all of the hospital employee benefits, FMLA, billing, filing, supplies, and payroll corrections (time and attendance).  Must be very detailed oriented and have excellent customer service skills. Small department, consists of 3 people.  Minimum of 2 years human resources experience, prefer hospital experience, knowledge of State and Federal HR laws, proficient in EXCEL and Microsoft Office. 

STAFF NURSE (PRN) - Inpatient Care Unit
We need RNs who can fill in days and nights for IPCU.   We are looking for experienced surgical, med-surg, recovery, pre-post nurses to expand our PRN Pool. As full-time positions open, we will make every effort to promote from this Pool of nurses. Current RN licensure in the State of Texas, CPR, and ability to obtain ACLS certification within 6 months of hire required.  Orthopedic experience preferred

Responsible for ensuring the accuracy and completeness of inpatient and outpatient clinical coding; validates accuracy of coded information and MS-DRG assignment to ensure compliance and proper facility reimbursement. Qualifications for the position include: CCS required, ICD-10-CM education required, and Minimum 2 years of IP/DRG coding experience.

Minimum 2 years previous hospital experience required. Surgery experience preferred. Must be able to function in Pre-op, Post –op and the surgical setting.

Must have RVT registry or CRT.

STAFF NURSE (FT) – Operating Room/Recovery at Golden Road facility
Progressive Outpatient Surgical Unit utilizing the shared governess model. Full-time Operating Room/Pre-Post RN. Minimum of 3 years recent OR experience required. Will work in OR, PACU, and Pre-Op. Must be able to work as early as 5:30am or stay till 6:00pm depending on the unit worked. No weekends, no call and no holidays.
